Case Management

CARE has several case management programs which you may participate in. These programs link individuals with a case manager who is available to provide support and access to resources and services in the community. Each of our case management programs sets out to work with a specific population and has different eligibility requirements. Targeted case management programs include parent education, methadone treatment, substance abuse support services, and women and families.  These services are free to those who qualify and are made possible by grants and contracts CARE holds with community partners.  Please call 586-541-CARE for more information.
